W.S. 9-1-507 (a) (vii) requires the submittal of Survey of Local Government Finances form to the Department of Audit by September 30, each year following the end of the fiscal year.
To complete the Survey of Local Government Finances form online, you will need a user name and password from our office. If you have misplaced the user name and/or password, send an email to doa-pfd-web@wyo.gov with “Login Issues” in the subject line, and we will provide that information to you.
New organizations will need to request login information, please send an email to doa-pfd-web@wyo.gov including the name of the entity, your affiliation with the entity along with your name and phone number.
Additional Paperwork is required. All entities are required to submit the Annual Report Summary. All other follow-up paperwork will be based on the total revenues and expenditures of the entity. These forms may be found here. Please email all paperwork to: doa-pfd-web@wyo.gov.
